With the advancement of neoadjuvant therapies, some patients achieve clinical complete response (cCR) following chemoradiotherapy, opening the door to non-operative management and the possibility of avoiding a permanent stoma. However, concerns over local regrowth, subjective response assessment, and the psychological burden on patients continue to challenge the broader application of this approach.
