Combining PD-1 Monoclonal Antibody and HDAC Inhibitor with Conventional P-Gemox Significantly Improves the Treatment Outcome for NK/T Cell Lymphoma

From June 17th to 23rd, 2023, the 17th International Conference on Malignant Lymphoma (ICML) was held grandly in Lugano, Switzerland. As the world's largest conference on malignant lymphoma, ICML has become an essential event in the field of lymphoma research and treatment over the past decades, aiming to present and discuss the latest basic, translational, and clinical research on lymphoma. NK/T cell lymphoma is a relatively rare hematological malignancy with no standardized treatment regimen. At this conference, Dr. Huiqiang Huang from the Cancer Center of Sun Yat-sen University,China,  gave an enlightening lecture on the treatment strategy for NK/T cell lymphoma. Extranodal NK/T-cell lymphoma (ENKTCL) of nasal type is a rare subtype, commonly found in the Asian population and associated with the Epstein-Barr Virus (EBV). At present, the prognosis for patients remains poor, especially for relapsed/refractory (R/R) cases. From June 8 to 11, 2023, the 28th European Hematology Association (EHA) annual meeting was held in Frankfurt, Germany. A selected oral report (Abstract No.: S226) from the conference showed that the use of PD-1 inhibitors alone or in combination significantly improved the treatment of R/R ENKTCL patients. This study further confirmed that immune checkpoint inhibitors have become an important treatment option for R/R ENKTCL patients. Intravascular Large B-Cell Lymphoma (IVLBCL) is a subtype of large B-cell lymphoma, characterized by large tumor B-cells aggregating inside blood vessels. This disease is extremely rare and highly malignant, often involving the central nervous system, lungs, and skin. Its prognosis is poor and clinical treatment challenging. At the recently concluded 28th European Hematology Association (EHA) Annual Meeting, a study (Abstract Number: P1185) from Dr. Daobin Zhou's team at Peking Union Medical College Hospital was selected. This study evaluated the efficacy and safety of the Zanubrutinib combined with R-CHOP regimen in treating initial IVLBCL, attracting widespread attention. To further understand this study, the Oncolgy Fronter invited Dr. Zhou for a detailed interpretation and analysis, summarized as follows: IVLBCL is a rare type of aggressive B-cell lymphoma that does not form a tumor mass. Its diagnosis is difficult and lacks an established standard treatment. Zanubrutinib is a novel Bruton kinase inhibitor. Recent studies have shown that Zanubrutinib has good therapeutic effects in B-cell lymphomas originating from activated B-cells, those with dual expression, and those with MYD88 mutations.
