In February 2025, the Journal of Hepatology published a pivotal study revealing how the Freiburg Index of Post-TIPS Survival (FIPS) can effectively identify patients at high risk of further decompensation and acute-on-chronic liver failure (ACLF) following transjugular intrahepatic portosystemic shunt (TIPS) implantation.
Key Findings:
High-risk FIPS patients are significantly more prone to developing further decompensation and de-novo ACLF after TIPS implantation.
While low-grade ACLF at the time of TIPS placement may resolve, ACLF recurrence is notably more frequent among FIPS high-risk patients.
Decompensation and ACLF post-TIPS are major drivers of mortality, contributing to significantly reduced transplant-free survival.
The study, involving 1359 cirrhotic patients across eight tertiary centers, underscores the independent predictive power of FIPS in identifying patients likely to experience adverse outcomes. These findings pave the way for more tailored clinical management strategies to improve survival outcomes in high-risk patients.
